Draft, then approve: our rule for AI in clinical notes

Every AI-generated note, report, or extracted field passes through a draft → human-review → approve state machine and is recorded in an append-only audit log. AI never auto-finalizes into the clinical record. This is enforced in application code on top of the database, and remains non-negotiable regardless of which AI provider we connect.
